Be Careful With Drug "Quick" Codes

A recent "near miss" will put more focus on selecting the right drug from the computer and pharmacy shelves.

In this latest case, topiramate 25 mg and lamotrigine 25 mg tabs were accidentally stocked together because their packaging looks almost identical...and BOTH were used to fill ONE prescription.

We're all familiar with the reasons for these mix-ups...look-alike/sound-alike meds, multiple dosage forms or salts, etc.
