Mix-ups are still occurring with meds that come as both solids and liquids...but you can help prevent them.

Read Rxs closely. For example, "sol" or "susp" written on an Rx can easily be missed or mistaken for tabs instead.

Clarify Rxs that don't have a dosage form. An Rx for "amoxicillin 250 mg" could be for the chewable tablet, capsule, or suspension.
