Substitution of long-acting calcium channel blockers (diltiazem, nifedipine, etc) will continue to cause confusion.

These heart meds come in multiple formulations and go by many different names. For example, diltiazem extended-release capsules come as Apo-Diltiaz CD, Teva-Diltiazem ER, Tiazac, etc.

But some of these are equivalent...and some are not.
