Stay Alert for Patients Getting Two Oral Anticoagulants

Should patients ever get BOTH warfarin and a direct oral anticoagulant (DOAC), such as Eliquis (apixaban) or Xarelto (rivaroxaban)?

Rarely. Both of these oral anticoagulants thin the blood...and taking them together may increase the risk of life-threatening bleeding.

Consider it a red flag if you see a DOAC with warfarin...and notify your pharmacist. They'll need to confirm whether the overlap is intentional...or a mistake.
