DEXA (dual X-ray densitometry) tests are the gold standard to detect osteoporosis. They measure bone density of the hip and spine.

But traditional DEXA tests are too expensive for routine screening. That's why many patients don't find out they have osteoporosis until AFTER they've had a fracture.
